150% Rule - Special rate for PPO Plans composite rate only for husband and wife
employed at the same District
Provides each District or Bargaining Unit with a choice of up to
four PPO plans and three HMO plans
Provides Delta Dental, Vision Service Plan, Employee 
Assistance Program and Group Term Life Insurance

On prescription drug program, unless the physician requires the
use of a brand name drug, prescriptions will be filled with 
a generic equivalent when available

CVT uses a Prudent Buyer Chiropractic network;
no forms, no hassles for you. There is no limit to the number
of medically necessary visits for a preferred provider.
Limitations apply for a non-participating provider.

All eligible retirees must enroll in Medicare Part B;
those eligible for Part A must enroll in Part A
Dependent children are covered to age 19 or to age 25
(through age 24)
if a full time student at an accredited 
institution, OR if claimed on parents federal tax return
CVT has NO pre-existing condition clauses
