California's Valued Trust is pleased to provide a valuable service that provides eligible members with chronic conditions a personal healthcare support system. This program, called AccordantCare, is designed to help meet unique healthcare needs of California's Valued Trust members.
AccordantCare is a personalized health management program designed to help you feel your best. It's voluntary, confidential - and best of all - it costs you nothing. That's because it's already paid for as part of your health benefits if you are eligible.
This sophisticated disease management program is designed to help CVT members with specific common chronic medical conditions:
- Asthma (Adult & Pediatric)
- Chronic Obstructive Pulmonary Disease (COPD)
- Coronary Artery Disease (CAD)
- Peptic Ulcer Disease (PUD)
- Diabetes
- Depression
- Hypertension
- Musculoskeletal Chronic Pain (MSCP)
- Headache
Helping Participants Improve Their Health
The program specifically supports eligible CVT members by providing them with information about active and effective self-management while helping providers to educate their patients and to promote adherence to the treatment plans. Members participating in the program have access to specialized nursing support, which is available by telephone 24 hours a day, 7 days a week along with monthly newsletters with preventive tips and a Web site containing in-depth, condition-specific information with an extensive archive of health resources.
