CVT's Health Management Program
Health Management Defined
California's Valued Trust is pleased to provide a valuable service that provides eligible members with chronic conditions and rare disease states a personal healthcare support system. The programs are designed to help meet the unique healthcare needs of California's Valued Trust members. These personalized health management programs are designed to help members feel their best. They're voluntary, confidential - and best of all - it costs members nothing. That's because it's already paid for as part of their health benefits if they are eligible.
Helping Participants Improve Their Health
These programs specifically support eligible CVT members by providing them with information about active and effective self-management while helping providers to educate their patients and to promote adherence to the treatment plans. Members participating in the program have access to specialized nursing support, which is available by telephone 24 hours a day, 7 days a week along with monthly newsletters with preventive tips and a Web site containing in-depth, condition-specific information with an extensive archive of health resources.
Alere Health Management Program for Chronic Diseases
The chronic program is provided by Alere. This sophisticated health management program is designed to help eligible CVT members who have specific chronic medical conditions:
