Kaiser HMO Plans
Description
With many locations throughout California, Kaiser allows CVT member units who choose Kaiser as their HMO provider to select their own personal Kaiser doctor. Many Kaiser facilities offer a wide range of services like primary care, lab, X-ray, and pharmacy all under one roof, saving participants extra trips.
CVT offers eight Kaiser HMO plans in addition to a new HMO Plan with Wellness Program. A unit can offer up to two of the eight HMO plans and the HMO Plan with Wellness Program. Each unit can choose to offer Kaiser as long as the participants live or work in an approved Kaiser zip code area.
The plans range from those with no co-pays up to $25 co-pays for doctor visits. One plan offers no out of pocket costs while others require per person and per family a set maximum. Outpatient X-ray and Lab are covered under all plans at no charge. There is no lifetime maximum amount. Prescription drugs benefits range from a co-pay of $5 in plans 1 and 2 up to $10 for generic and $30 for brand name drugs in Plan 7. There are no vision or chiropractic benefits with the CVT Kaiser plans.
