CVT’s Health Management Programs
Helping Members Improve Their Health
California's Valued Trust (CVT) is dedicated to providing up-to-date healthcare information and services in addition to its comprehensive plan benefits. Currently there are two health management programs available from CVT. They are Anthem ConditionCare and Accordant - read more and see if you can benefit from one of these resources available at no cost to you.
The Anthem ConditionCare program supports Anthem members who have certain common chronic conditions. Anthem’s ConditionCare health and wellness program is available for active and non-Medicare retiree subscribers and covered dependents on CVT’s Anthem PPO and HMO health plans.
If you or a covered family member have one of the long-term (chronic) health conditions listed below, Anthem ConditionCare is for you. It’s personal. It’s private. And it’s available at no additional cost. ConditionCare provides tools, resources and support for members who have:
- Asthma (pediatric and adult)
- Chronic obstructive pulmonary disease (COPD)
- Coronary artery disease (CAD)
- Diabetes (pediatric and adult, types 1 and 2)
- Heart Failure (HF)
In addition, a member, spouse or domestic partner may also be eligible for reduced pharmacy co-payments for certain condition-related prescriptions when appropriately enrolled in one of the approved programs listed above. This does not apply to members enrolled in Medicare/Silverscript.
If you have one of the conditions listed above, please be aware that you may receive a call from an Anthem nurse inviting you to join the program. Or you can also join anytime by calling Anthem ConditionCare toll-free at 800-234-4333 and reference "ConditionCare" when calling in. They are ready to support you!

AccordantCare® Health Management Program
Accordant Health Services, a CVS/caremark company, provides care management to Anthem and Blue Shield of California PPO members with complex, chronic conditions. It is designed to help members find the answers and support they need to manage their health care needs and maximize their overall health status. It is voluntary, confidential - and best of all - it's available at no additional cost to eligible members.
The AccordantCare program is designed for members with:
- Amyotrophic Lateral Sclerosis (ALS)
- Chronic Inflammatory Demyelinating Polyradiculoneuropathy (CIPD)
- Crohn's Disease
- Cystic Fibrosis (CF)
- Gaucher Disease
- Human Immunodeficiancy Virus (HIV)
- Systemic Lupus Erythematosus (SLE or Lupus)
- Multiple Sclerosis (MS)
- Myasthenia Gravis (MG)
- Parkinson's Disease (PD)
- Rheumatoid Arthritis (RA)
- Epilepsy (Seizure Disorders)

Living with a long-term health condition can be hard at times. We know our members want and need different resources and information to help you live well. That's why CVT is working with Accordant Health Services to offer the AccordantCare Program.
Whether you're dealing with a long-term condition yourself, or you're taking care of someone who is, sometimes you may need to talk with someone or ask a question about health problems. Other times you may need a helping hand to fix a problem you're having, to get the care you need, or to learn something you did not already know. No matter how much or what kind of support you need, the AccordantCare program is here to help you.
A team of nurses is waiting to talk with you and answer your questions by phone or email, 24 hours a day, 7 days a week. The program gives you monthly newsletters filled with the latest news about treatments and medicines, tips for managing stress, and other useful information. You also have access to www.accordant.com, where you can find helpful articles, join webinars, watch videos about staying healthy, and download tips and tools you can take with you to your next doctor visit.
The AccordantCare program can:
- Provide tips on how to stay well.
- Help you find ways to save money.
- Help you find ways to put your doctor's advice to work in your busy life.
- Help you connect to resources near your home.
In addition, a member, spouse or domestic partner may be eligible for reduced pharmacy co-payments for certain condition-related prescriptions when enrolled and engaged with a nurse in an approved AccordantCare health management program. Ask your Accordant representative if your prescription(s) are eligible. This does not apply to members enrolled in Medicare/SilverScript.
