IMPORTANT UPDATE: Members Taking a GLP-1 Medication for Weight Loss

Effective October 1, 2025:

• GLP-1's will only be covered for members who have a starting Body Mass Index (BMI) of 35 or greater
• Limited to 30-day supply
• All members using GLP-1 for weight loss will require a new authorization
