Joining a new group healthcare plan can be a challenge. New identification cards, new forms, new contacts. And being a member of CVT brings you many different partners who are leading carriers in the healthcare industry. As a result, you may see many different products and provider names associated with your benefits, but it’s important to note they are your partners through CVT. It can be confusing and sometimes overwhelming. At CVT we understand that, so we have a whole team of very skilled staff members who are here to help you with any and all questions you might have. 1. Getting Started - Enrolling

If you have not already done so, you should complete a new CVT enrollment form provided by your district/chapter or enroll via MyCVT.cvtrust.org, our online enrollment portal. If your district has chosen to support the use of MyCVT, you can save a lot of time and make it convenient for yourself by enrolling in MyCVT. You can have on-demand information about eligibility, benefit enrollment, and document sharing through a single online service between you, your district and CVT. 2. Selecting a Plan

Your district/chapter has already selected several plan options that you can choose from. See your district benefits representative to determine which plans are available to you or if you are enrolled in MyCVT you can view your plan options there.

Plan Document or Evidence of Coverage (EOC) – EOCs are documents that explain the benefits, coverage and out-of-pocket costs of the different medical, prescription, and dental plans available through CVT. Summary of Benefits and Coverage (SBC) – The Patient Protection and Affordable Care Act requires by law that all new enrollees in a healthcare plan must have access to an outline of their plan’s coverage and benefits called a Summary of Benefits and Coverage (SBC). SBCs summarize essential healthcare information in a standard format to help you choose a health plan. Medical and Pharmacy Benefit Calculator - Which medical plan is right for you? – Good question. There are a number of factors to consider including what your co-pay and your annual deductible are, how many times a year you visit a provider and the prescriptions you and your eligible dependents have. Beacon Health Options Employee Assistance Program (EAP) - EAP is provided at no cost to all CVT subscribers with medical coverage. Employees and their family members can receive free, confidential assistance to help with personal and professional goals, manage daily stresses and develop fulfilling relationships. ConsumerMedical® - As active and non-Medicare retirees, you and your eligible dependents have access to ConsumerMedical® for free, confidential, ono-on-one support for any medical condition.  They provide information to help you better understand your diagnosis and treatment options.  They offer recommendations for the best local, in-network doctors and hospitals and can help you get a second opinion when you need one.  For more information click on the link below:

Fit for Life Wellbeing Program - This is a CVT initiative designed to provide all of our members with healthy lifestyle opportunities. CVT, in partnership with our vendors, provide health and wellness services including district-wide, on-site biometric health screenings, flu shot clinics and seminars on various topics. MDLIVE® - As a PPO plan member, you can use MDLIVE® for around-the clock on-demand access to a national network of board certified doctors (including pediatricians and dermatologists) who can diagnose, recommend treatment, and prescribe medication (if appropriate) for non-emergency medical conditions by secure video or phone.
